Before We Meet:
Please download my Policies Form, which explains my office procedures and offers an agreement for psychotherapy services. This includes important information about your confidentiality. If you have questions after reading this form, please bring them up when we meet.
My Social Media Policy explains my policies and procedures as they related to our potential interactions on the Internet. After you download this form, please let me know you have any questions or concerns about these policies.
I am required by law to provide you a copy of the HIPAA Notice of Privacy Practices, so you can understand your rights and protections related to the use and disclosure of your identifiable health care information. This form is available for you to download for your records.
What To Bring Prior to Our First Appointment:
Prior to our first session, please complete my Patient Data Form, and bring it to our first appointment. This will help you share important details with me without having to arrive to your appointment early to complete paperwork.
After you have read my Policies Form, please sign the last page.
Once you have read the Social Media Policy, and the HIPPA Notice of Privacy Practice form, please sign my Acknowledgment Form. This states that you have access to, and have read all forms.
